Mindoula is seeking to hire a talented Director, Population Health, North Carolina, to lead our new and fast-growing business line in this state. The Director of Population Health will lead Mindoula’s Population Health Program in North Carolina to serve SMI/SUD members in the Raleigh-Durham market and around the region in an innovative, value-based deal being launched with a major health plan partner.

The ideal candidate will have an entrepreneurial spirit, meaningful population health management experience, and thrive in a high-growth environment. They will roll up their sleeves, build a team of Care Extenders, and be responsible for the oversight of service within the Population Health Program. A combination of experience with behavioral health management and team leadership skills is required to be successful.

Essential Duties and Responsibilities:

  • Implement the Population Health program in partnership with the General Manager, North Carolina, and other members of the North Carolina leadership team
  • In partnership with the Talent Acquisition team, recruit, hire, onboard, train, and retain talented, high-performing, and compassionate care extenders (case managers)
  • Understand contractual obligations to the health care plan partner in full, and ensure that all are met successfully at all stages of program implementation and delivery.
  • Monitor team performance to ensure all appropriate services are being provided to members in order to meet/exceed the overall goals of the program
  • Provide leadership to ensure the Population Health Program team delivers the highest quality of culturally-centered care across all settings: virtual and in-person/home, for members of all ages, backgrounds, and diagnoses
  • Problem-solve, as needed throughout the program launch to ensure all issues, are resolved, goals are met/exceeded, and key partners and stakeholders are satisfied
  • Model a hands-on approach as the leader of a community-based team, empowering your team to meet members where they are and tapping into the wide range of local resources to deliver whole-person care
  • Act as a player/coach, working alongside Care Extenders to build their skills in everyday situations and to directly support members in more challenging situations or as needs arise

Education & Work Experience:

  • A Qualified Mental Health Professional (QMHP) holding one of the following licenses: Licensed Clinical Social Worker (LCSW), Clinical Professional Counselor (CPC), Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), Licensed Psychologist (Ph.D. or PsyD), or equivalent.
  • Five or more years of population health management experience/healthcare management in a managed care setting experience preferred
  • Three or more years of experience managing/leading teams
  • Hands-on case management experience and experience leading a case management team, including community-based visits
  • Experience leading, building, and implementing programs with a value-based performance component
  • Experience working with and understanding health care data - claims, utilization, diagnoses, treatments
  • Work experience with EHRs and the ability to work with multiple simultaneously
